Before any supplies can be ordered and shipped the Enrollment Form must be completed and we must obtain a prescription from your physician. Orders can be placed by calling 1-888-678-6692 Monday through Friday, 8:30am to 5:00pm Central Standard Time

At Duke Medical Supply we specialize in handling the Medicare paperwork and billing.

Ostomy Supplies: We accept Medicare Assignment on most ostomy orders, which means: There will be little or no up-front cost, depending on your Medicare supplemental insurance. If you have Medicare supplemental insurance the entire cost of your supplies may be covered.

Urological Supplies: We accept Medcare Assignment on some items, meaning little or no payment is required up front. On other items payment up front is required and Medicare will send any reimbursement to you. We accept Visa, MasterCard, American Express, Personal Checks, and Money Orders.

With your permission we will contact your physician and obtain the prescriptions required by Medicare.

We accept Medicare Assignment and provide free standard shipping on most ostomy supplies.

On unassigned orders, standard shipping is free on orderes over $95.00. For orders under $95.00, there is a nominal shipping fee.

You may request plain packaging for your shipments at no extra charge.

Returns and Warranties:	Return to Top of Page

Please call or e-mail for authorization prior to returning any merchandise. Unopened, unused items in Like-New condition, or any defective merchandise may be returned for refund, credit or replacement within 90 days of purchase, except items marked non-returnable. Shipping charges are not reimbursed.

Duke Medical Supply, Inc. will honor all manufacturer warranties. Please call us at 1-888-678-6692 or e-mail us for a Return Authorization.

Please note: Return Authorizations are good for 30 days from the date they are issued.
